Developing Safety-Net Care Management in California: An Opportunity for Whole-Person Care

Care management is becoming increasingly established as an approach for improving care and outcomes for complex, high-need patients and controlling health-systems costs. Most care management has been designed and delivered at an organizational level through either providers or payers. Developing approaches for the safety net at a county level offers the potential to engage more partners and increases potential resources to respond to a wider range of whole-person needs.
