JSI has partnered with Social Impact Exchange to provide sustainability technical assistance to the eight grantees in the Merck Foundation's Bridging the Gap (BtG) initiative. BtG focuses on developing cross sector partnerships between healthcare, behavioral health, social service, and other sectors to address the health and social factors driving disparities in diabetes rates and outcomes. JSI will be working with the sites to clarify their programmatic model and partnerships, develop a value proposition for their collaborative, identify and investigate potential funding models, and ultimately secure resources necessary to maintain and/or scale up their efforts.
