Colorado State Innovation Model (SIM) Practice Facilitation

Provided practice facilitation through the Colorado State Innovation Model (SIM) to four practices in Colorado around integrating behavioral health into primary care, as well as transforming their services to support the achievement of the quadruple aim (better care, happier patients, lower costs and more satisfied providers and staff). Worked with the University of Colorado and other key stakeholders throughout the state.
