Insights and Tools for California Health Homes Implementation

JSI has been working with key stakeholders since 2011 to advance a Section 2703 State Plan Amendment and Health Homes program for high-risk Medi-Cal and dual eligible beneficiaries in California. As the initiative moves into implementation, we will provide on-going analysis of program elements; monitoring of implementation progress and distillation of challenges and best practices, and research and tools to support on-the-ground leaders.
