We worked on the development of a project plan for the Integrated Delivery Network serving Region 5 of New Hampshire's Delivery System Reform Incentive Payment (DSRIP) program – CMS 1115 Medicaid Waiver for Health System Transformation. Our work involved the completion of a behavioral health needs assessment and development of project plans for integrated care, health information technology (HIT) development, workforce development and three community projects for substance use disorder (SUD) service expansion, enhanced care coordination, and community re-entry for incarcerated individuals.
